Responsibilities
- Provide case management and care coordination for acute inpatient patients across multiple units.
- Conduct utilization review, discharge planning, and coordination of post-discharge services to support safe transitions.
- Apply DRG/charge knowledge to support appropriate documentation and billing processes.
- Collaborate with interdisciplinary teams (IV teams, PT/OT, radiology, social services, pharmacy, lift teams) to optimize patient outcomes.
- Document patient progress and care plans in Epic; communicate changes to the care team and family members as appropriate.
- Support unit operations, including admissions, transfers, and timely patient flow; participate in rapid response when needed.
- Ensure compliance with hospital policies and state regulations; maintain patient safety and privacy standards.
Requirements
- Active RN license in Washington (WA) or a CompactState license
- BLS certification required
- Minimum 2 years RN experience; acute care hospital experience required and recent (within last 3 years)
- Previous charge nurse experience required
- Epic charting experience required
- Ability to work in inpatient acute care setting, across assigned units
- Strong care coordination, discharge planning, and utilization review skills
- Ability to float within scope to meet facility needs; comfortable with 5x8-hour shifts and EOW scheduling
- First-time travelers accepted with strong acute care background
- Locals accepted: No (50-mile local radius rule)
