Responsibilities
- Conduct comprehensive PT evaluations and develop individualized treatment plans for assigned patients.
- Implement evidence-based interventions (therex, manual therapy, modalities) and progress as appropriate.
- Reassess patient progress and adjust treatment plans to achieve functional goals.
- Document assessments, treatments, and outcomes in the electronic medical record in a timely, compliant manner.
- Collaborate with an interdisciplinary team to coordinate care and plan discharges.
- Educate patients and families on home exercise programs, safety, and recovery expectations.
- Adhere to billing procedures and ensure all required documentation is uploaded according to policy.
Requirements
- Active Physical Therapist license in Illinois (or eligible to practice in Illinois)
- Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program
- BLS certification or equivalent
- 1 year of clinical PT experience (travel experience preferred)
- Ability to work daytime hours and fulfill a 40-hour workweek
- Demonstrated excellence in communication and documentation; ability to follow all uploaded requirements and billing procedures
