Responsibilities

- Document assessments, plans, and outcomes in the health record; ensure regulatory and payer guideline compliance
- Communicate with patients and families about plan of care and facilitate smooth transitions of care
Requirements
- Current California RN license
- Associate Degree in Nursing required; Bachelor's degree in Nursing preferred
- BLS certification (American Heart Association) required
- 3–5 years of nursing experience in an acute care setting
- At least 1 year of hospital care management experience; recent utilization review, discharge planning, or inpatient case management preferred
