Responsibilities
- Conduct comprehensive PT evaluations for residents and document functional status, mobility, and goals using approved forms.
- Develop and implement individualized treatment plans focusing on gait training, balance, range of motion, strength, and pain management; adjust as progress or barriers arise.
- Deliver direct therapy sessions on a daily basis, incorporating appropriate modalities while adhering to safety and evidence-based practice.
- Reassess residents regularly, track progress, and communicate updates to the interdisciplinary team to refine care plans.
- Educate residents and families on home exercise programs, fall prevention strategies, and approaches to maintain independence after discharge or return to the community.
- Maintain accurate, timely documentation and comply with privacy, regulatory, and facility policies.

Requirements
- Active Montana physical therapy license or eligibility for licensure
- Degree in Physical Therapy from an accredited program (DPT preferred)
- Experience or comfort working with older adults in a skilled nursing/geriatric setting preferred; new graduates considered with appropriate supervision
- Strong communication and collaboration skills to work within an interdisciplinary team
- Ability to commit to a day-shift schedule and 36 hours per week for a 13-week assignment
- Compliance with licensure and regulatory requirements applicable to a physical therapy practice in a long-term care setting
