Responsibilities
- Provide direct, compassionate nursing care to residents in a long-term care setting, including monitoring vitals, assisting with activities of daily living, and basic nursing procedures under supervision.
- Administer medications and treatments per facility policy; document administration and response in the charting system; report deviations or concerns to the supervising nurse.
- Monitor residents for changes in condition, recognize deteriorations, and communicate findings promptly to the care team.
- Support care planning and participate in interdisciplinary rounds to update goals of care and resident outcomes.
- Promote safety, infection control, privacy, and resident dignity in all caregiving activities; assist with mobility and restorative therapies as directed.
- Complete required documentation accurately and timely, ensuring compliance with regulations and facility standards.

Requirements
- Active LPN license in New Hampshire (in good standing)
- Graduate of an approved practical nursing program
- At least 1 year of LPN experience in a long-term care or skilled nursing facility (preferred)
- Basic Life Support (BLS) certification required
- Ability to work 12-hour day shifts and meet the demands of a travel assignment, including pre-employment screening and credentialing
