Responsibilities
- Conduct initial and ongoing physical therapy evaluations for residents within the long-term care setting and develop individualized treatment plans.
- Implement evidence-based therapeutic interventions to address mobility, strength, balance, gait, and functional goals.
- Reassess progress regularly, adjust plans of care, and document outcomes in accordance with policies and payer requirements.
- Collaborate with nursing staff, therapy team, and supervising clinicians to coordinate care, ensure safety, and support transition planning.
- Maintain accurate, timely documentation and adhere to HIPAA and facility policies for patient records and billing.

Requirements
- Active or eligible-for-licensure physical therapist license in MT (state-specific licensure required for assignment).
- Degree from an accredited physical therapy program (Doctor of Physical Therapy or suitable PT degree).
- Experience in geriatric or long-term care settings preferred; strong communication and teamwork skills.
- Ability to work in a long-term care environment and to participate in a day shift schedule.
- Willingness to travel to the assignment location and commit to the 13-week duration.
- CPR/BLS certification as applicable per facility policy (if required by local guidelines).
