Responsibilities
- Perform evaluative and therapeutic procedures under physician order, observing precautions, indications, and contraindications.
- Provide consultation to referring physician and interdisciplinary team regarding plan of care and discharge needs; attend relevant team meetings.
- Monitor progress, adjust treatment plan as needed; document all evaluative and therapeutic interactions in the Electronic Medical Record with clear, accessible language.
- Educate residents and caregivers on therapy goals and use of assistive devices; supervise assistant-level staff as appropriate; ensure compliance with Medicare/Medicaid and other reimbursement guidelines.
- Maintain infection control, participate in in-service education, and contribute to program development and quality improvement within Rehabilitation.
Requirements
- Active Physical Therapist license in Massachusetts.
- Bachelor's degree in Physical Therapy required; Master's or Doctorate preferred.
- Prior skilled nursing facility / long-term care experience preferred; must have SNF experience.
- Basic Life Support (BLS) certification preferred.
- Strong communication, documentation, and teamwork skills; proficient with Electronic Medical Records and Microsoft Office.
