Responsibilities
- Administer medications and document in patient records according to policy
- Monitor vital signs, perform focused assessments, and report changes to the supervising nurse

- Collaborate with RNs and other members of the care team to implement and update care plans
- Document accurately in the electronic health record, maintaining compliance with privacy and safety standards
Requirements
- Active LPN license in West Virginia (or eligibility to practice in WV) and in good standing
- CPR/BLS certified
- Ability to work 12-hour shifts and commit to a 14-week assignment
- Experience in long-term care or geriatric care preferred
- Strong communication and teamwork skills; reliable documentation and adherence to care plans
