Responsibilities
- Administer prescribed medications and treatments per orders and nursing protocols
- Monitor resident condition, document vitals, and report changes to the supervising RN
- Provide routine nursing care and assist with activities of daily living (ADLs)
- Perform wound care and dressing changes as assigned, following facility policies
- Maintain accurate charting and documentation; communicate with the interdisciplinary team
- Ensure infection control practices, safety, and privacy during care transitions and transfers

Requirements
- Active LPN license in good standing (Pennsylvania or eligible for licensure in PA)
- Relevant experience in long-term care or similar settings preferred
- Ability to work the stated evening shift (3:00 PM–11:30 PM) and participate in required rotation
- BLS for Healthcare Providers certification or equivalent
- Ability to pass background check and drug screen
- Graduation from an accredited practical nursing program
