Responsibilities

• Provide direct, resident-centered nursing care according to the individualized care plan
• Administer prescribed medications and document responses and health status
• Monitor vital signs, assess changes, and report findings to the supervising nurse or physician
• Assist residents with activities of daily living and support mobility and safety needs
• Maintain accurate, timely documentation in the electronic health record and care notes
• Collaborate with the interdisciplinary team to ensure continuity of care and quality outcomes
  
  

Requirements

• Active LPN license in Pennsylvania (in good standing)
• Nursing experience in long-term care or a similar setting is preferred
• Ability to work Day or Evening shifts with guaranteed 40-hour weeks
• Willingness to participate in a travel temporary assignment
• Strong communication and teamwork skills, with a resident-centered care approach
