Responsibilities

• Conduct comprehensive speech-language pathology evaluations for residents and create individualized treatment plans
• Provide therapy addressing speech, language, cognitive-communication, and swallowing disorders; implement targeted therapeutic activities
• Document progress notes, treatment plans, and discharge summaries in the resident’s chart; ensure regulatory and facility compliance
• Collaborate with an interdisciplinary team including nursing, rehabilitation professionals, and social services to optimize resident outcomes
• Educate residents, families, and caregivers on communication strategies and swallowing safety; participate in discharge planning as appropriate
  
  

Requirements

• Ability to work in a skilled nursing facility setting and within a resident-centered care model
• Strong communication, documentation, and teamwork skills
• Demonstrated clinical skills in evaluation and treatment of speech, language, and swallowing disorders
• Willingness to complete assignment as a travel clinician and comply with facility policies and timelines
