Responsibilities

• Provide comprehensive case management for a defined patient population from pre-admission through post-discharge, collaborating with the multidisciplinary team to optimize outcomes
• Act as a liaison among the patient, family, nurse, physicians, and other providers to coordinate and ensure smooth transitions of care
• Develop, implement, revise, and report on the case management plan; monitor progress and adjust as needed to meet quality and patient-centric goals
• Serve as a clinical resource to the care team on specific patient items, offering guidance on care plans, resources, and best practices
• Coordinate the continuum of care, ensuring timely progression through hospitalization and subsequent steps in the care pathway
• Support efficient patient and family flow through the planning and execution of care, discharge planning, and follow-up in the hospice/home health context
  
  

Requirements

• Active Registered Nurse license in California
• Relevant case management experience, preferably in hospice or home health settings
• Education: RN degree (ADN or BSN) required; BSN preferred
• Strong communication, coordination, and advocacy skills
• Ability to work within a home-based hospice/home health care environment and collaborate with a multidisciplinary team
