Responsibilities

• Deliver PTA services under the supervision of a licensed physical therapist to residents in a LTAC/long-term care setting
• Implement and monitor treatment plans including therapeutic exercises, gait training, mobility training, and activities of daily living tasks
• Assist with patient assessments, document progress, and communicate findings to the supervising PT and care team
• Ensure patient safety during therapy sessions, apply proper body mechanics, and adhere to infection control and safety protocols
• Collaborate with nursing staff and family members to support rehabilitation goals and discharge planning
• Maintain accurate, timely documentation in patient records according to facility policies and regulatory requirements
  
  

Requirements

• Active Physical Therapy Assistant license in New Hampshire or eligibility to obtain licensure per state regulations
• Associate degree from an accredited PTA program
• Ability to work in a LTAC/long-term care setting and adapt to a skilled rehabilitation workflow
• Strong communication and teamwork skills; comfortable documenting in electronic health records
• Previous experience in LTAC or skilled nursing environments preferred
• BLS/CPR as required by employer and facility policy
