Physical Therapist needed for a travel assignment in a skilled nursing facility setting in Moses Lake, WA. Responsibilities

• Perform initial evaluations and ongoing PT assessments for residents with mobility, balance, gait, strength, and functional limitations
• Develop, implement, and adjust individualized treatment plans to maximize functional outcomes and minimize fall risk
• Provide direct patient care, including therapeutic exercise, gait training, manual therapy, and modality-based interventions as appropriate
• Monitor and document progress, communicate changes to the interdisciplinary team, and participate in discharge planning and goal setting
• Ensure patient safety, maintain accurate records, and adhere to HIPAA and facility policies
• Collaborate with nurses, speech therapists, occupational therapists, and other clinicians to support comprehensive resident care
  
  

Requirements

• Active, unrestricted Physical Therapist license in Washington
• Doctor of Physical Therapy (DPT) degree from an accredited program
• CPR/BLS certification
• Prior experience in a skilled nursing facility or long-term care setting preferred
• Ability to work a 14-week travel assignment with guaranteed hours and a 5x8 day shift
• Excellent communication, documentation, and team-collaboration skills
