Responsibilities

• Perform comprehensive PT evaluations and develop individualized treatment plans for residents requiring physical rehabilitation
• Implement evidence-based interventions to improve mobility, strength, balance, gait, and functional independence
• Reassess progress regularly, update treatment plans, and document outcomes in resident records per regulatory standards
• Collaborate with interdisciplinary team members to coordinate care and optimize therapy scheduling
• Educate residents and families on exercises, safety, and strategies to maintain function after discharge
• Ensure patient safety, maintain a clean and organized treatment area, and adhere to documentation and infection control policies
  
  

Requirements

• Active Physical Therapist license to practice in Washington state
• Doctor of Physical Therapy (DPT) degree or eligibility to practice
• Relevant experience or comfort working in a skilled nursing facility / long-term care environment
• Ability to perform in a hands-on clinical setting within the stated shift (08:00–16:30)
• Excellent communication and teamwork skills; ability to document accurately and timely
