Responsibilities

• Conduct physical therapy evaluations and develop individualized treatment plans for residents in a skilled nursing setting
• Implement evidence-based therapies to improve mobility, strength, balance, gait, and functional independence
• Administer therapeutic exercises, manual therapy, gait training, and assistive device training as clinically indicated
• Monitor patient progress, document outcomes, and adjust treatment plans in collaboration with the interdisciplinary team
• Educate residents and families on home exercise programs, safety, fall prevention, and activity modification
• Maintain accurate documentation in medical records and ensure timely communication with supervising clinicians and the care team
  
  

Requirements

• Active physical therapy license in New Hampshire or license eligibility
• Doctor of Physical Therapy (DPT) degree from an accredited program
• Ability to commit to a 13-week travel assignment with guaranteed 35 hours per week
• Experience in a skilled nursing facility or geriatric rehabilitation setting preferred
• Strong communication and collaboration skills; able to work within an interdisciplinary team
• Eligibility to work in the United States without sponsorship (as required for this assignment)
