Responsibilities

• Evaluate and treat residents with physical therapy needs in a skilled nursing facility setting
• Develop and implement individualized treatment plans based on assessments
• Administer evidence-based therapies and monitor progress; adjust plans as needed
• Document patient care accurately in the charting system; communicate with the interdisciplinary team
• Educate residents and families on therapy plans, safety, and discharge planning

• Travel assignment with the ability to work in a skilled nursing facility setting
• Availability to start on 04/13/2026 and complete a 13-week contract
• Ability to work 35 guaranteed hours per week
• Shift availability aligned to 08:00–16:30, five days per week
