Responsibilities

• Conduct comprehensive patient evaluations and ongoing assessments to establish physical therapy goals for geriatric residents
• Develop and implement individualized treatment plans addressing mobility, strength, gait, balance, and functional independence
• Administer therapeutic exercises, manual therapy, gait training, and balance activities; monitor progress and adjust plans as needed
• Document patient status, progress notes, and outcomes in accordance with regulatory requirements and facility policies
• Collaborate with the interdisciplinary team (nursing, OT, SLP, and physicians) to coordinate care and communicate plans of care
• Ensure patient safety, infection control, and adherence to standard of care and regulatory guidelines within a long-term care setting
  
  

Requirements

• Active Physical Therapist license in New Mexico (in good standing)
• Graduation from an accredited physical therapy program (eligible for licensure if pending)
• Ability to work in a long-term care environment with older adults
• Availability for a 13-week travel assignment with 40 hours per week, Monday–Friday
• Strong communication, teamwork, and documentation skills
