Responsibilities

• Perform initial OT assessments in patients’ homes and develop individualized treatment plans to improve activities of daily living and functional skills
• Deliver OT interventions, including therapeutic activities and adaptive equipment training, to promote independence and safety in daily tasks
• Complete timely documentation for home health episodes, including evaluations, re-evaluations, discharges, and OASIS-related tasks as applicable
• Educate patients and family members on activity adaptation, safety strategies, and care techniques to support ongoing functional gains
• Coordinate with physicians, supervisors, and care teams to schedule visits, adjust plans of care, and ensure seamless transitions across services
• Maintain compliance with regulatory requirements and credentialing standards; ensure accurate maintenance of patient records and privacy
  
  

Requirements

• Active California Occupational Therapist license
• Basic Life Support (BLS) certification
• Valid driver’s license, auto insurance, and auto registration
• TB test (annual) and immunizations: MMR, Varicella, Hepatitis B, Tdap (10 years), Flu (annual)
• Ability to pass drug screening and physical examination
• Willingness to work in a home health setting and to travel as required
