Responsibilities

• Provide direct patient care for adults in a long-term care and rehab context
• Administer prescribed medications and document administration per policy
• Monitor vital signs, assess changes in patient condition, and respond appropriately
• Assist with care planning, documentation, and routine nursing tasks under supervision of the RN and multidisciplinary team
• Collaborate with physicians, therapists, counselors, and other professionals to support safety and recovery goals
• Maintain infection control practices and accurate charting in electronic health records
  
  

Requirements

• Active Vermont Licensed Practical Nurse (LPN) license
• Ability to work the overnight shift
• Willingness to travel for a temporary assignment
• Experience in long-term care or rehabilitation settings preferred
• Ability to work in both outpatient and inpatient rehab environments
• Must meet credentialing/safety requirements for travel assignments
