Responsibilities

• Provide direct patient care to long-term care residents per established care plans and policies
• Administer medications and treatments as prescribed; monitor and document patient status and changes
• Monitor vital signs, assess for adverse events, and respond promptly to changes in condition
• Collaborate with an interdisciplinary team to coordinate care and update care plans
• Assist with activities of daily living and promote resident comfort, safety, and dignity
• Complete accurate shift handoffs and maintain thorough clinical documentation; uphold infection prevention and standard precautions
  
  

Requirements

• Active Licensed Practical Nurse license in good standing
• Availability to work night shifts (23:00–07:30) and weekends
• Willingness to commit to a 13-week travel assignment and to travel to the assignment location
• Experience in long-term care or skilled nursing settings preferred
• Strong communication and teamwork skills; ability to adapt in a fast-paced environment
