Responsibilities

• Provide direct nursing care to residents per individualized plans of care
• Administer medications and treatments as prescribed, and document in the medical record
• Monitor resident status, report changes to the supervising RN, and respond promptly to safety concerns
• Assist residents with activities of daily living and support mobility, nutrition, and comfort
• Collaborate with the interdisciplinary care team to ensure coordinated, quality care
• Maintain regulatory compliance, complete accurate charting, and uphold infection control and safety standards
  
  

Requirements

• Active LPN license in Rhode Island (or eligibility for licensure by endorsement)
• CPR certification (BLS) preferred or required per facility policy
• Ability to work the listed shift and weekend requirements (7:00 am–3:30 pm with alternating weekends)
• Prior experience in long-term care or skilled nursing settings is preferred
• Effective communication and teamwork skills; strong patient-centered care mindset
• Willingness to comply with travel assignment requirements and onboarding processes
