Responsibilities

• Collect data on residents’ physical and psychosocial condition and communicate findings to the RN and Interdisciplinary Team
• Contribute to an age-appropriate, culturally relevant care plan reflecting nursing diagnoses and interventions, involving the resident/family and healthcare team
• Recommend revisions to the plan of care after outcome evaluation and coordinate referrals as needed; collaborate within the nursing scope
• Provide direct and indirect skilled nursing care under RN supervision; participate in mentoring and orientation of staff; maintain current nursing knowledge through education
• Administer medications and treatments per policy, monitor patient responses, and document appropriately
• Maintain safety, infection prevention, and environmental safety; support geriatric residents and promote a safe care environment
  
  

Requirements

• Current, active NY Licensed Practical Nurse licensure
• BLS certification (American Heart Association or RQI)
• Minimum 1 year of LPN experience in a long-term care setting
• Ability to perform rotations on D/E/N/W schedules and participate in holiday coverage
• Demonstrated ability to provide care for adult and geriatric residents and work effectively with an interdisciplinary team
• Must be able to start orientation on a Tuesday or Thursday and fulfill the 5x8 evening shift schedule
