Responsibilities
- Provide direct and indirect skilled nursing care for assigned residents under the supervision of a Registered Nurse
- Collect data on residents’ physical and psychosocial status; document and communicate findings to the RN and the Interdisciplinary Team
- Contribute to development and ongoing refinement of an age-appropriate, culturally relevant care plan involving the resident and family
- Recommend revisions to the plan of care and coordinate referrals to ensure comprehensive management of residents’ needs
- Maintain current nursing knowledge and competencies; participate in orientation, in-service programs, and mentoring of staff; support a safe, collaborative care environment
Requirements
- Active New York LPN licensure
- BLS certification (through American Heart Association or RQI)
- Minimum 1 year of LPN experience, preferably in a long-term care setting
- Ability to work nights and weekends/holidays as required
- Effective communication and teamwork skills; ability to care for adult and geriatric residents
