  • Cascade Park Garden’s New Horizon Program is a leading healthcare provider specializing in compassionate and comprehensive care for patients with behavioral health and dementia needs. Our team is committed to delivering high-quality, patient-centered care in a supportive and therapeutic environment. We are looking for dedicated and skilled LPNs to join our Behavioral Health and Dementia community, where your expertise and empathy will make a significant impact on the lives of our patients and their families.

     

    As an LPN with our New Horizon program, you will play a crucial role in providing specialized care to patients with behavioral health challenges and dementia. You will be responsible for assisting with planning, implementing, and evaluating the nursing care provided to ensure the highest standards of resident safety and well-being. Your ability to manage complex cases, provide emotional support, and collaborate with a care team will be essential to the success of our community.

     

    • - Ability to work autonomously as well as collaboratively with the support of the DRS, Staffing Care Coordinator, Resident Care Manager and care staff (med techs and resident aides).  
    • - Assess daily patients’ physical, emotional, and mental health needs.
    • - Collaborate with our team to develop, implement, and evaluate individualized care plans with residents’  families, and the healthcare team.
    • - Administer medications, and diabetes management, monitor patient responses, and adjust care plans as necessary.
    • -Supervise CNA’s, HCA’s and other care staff.
    • - Provide therapeutic interventions and crisis management for patients experiencing behavioral health challenges.
    • - Assist patients with activities of daily living, always ensuring dignity and respect.
    • - Educate patients and families about the management of dementia and behavioral health conditions.
    • - Maintain accurate and timely documentation of patient care in accordance with facility policies and regulatory standards.
    • - Collaborate with physicians, therapists, social workers, and other healthcare professionals to ensure a holistic approach to patient care.
    • - Participate in community meetings, continuing education, and quality improvement initiatives.
    • - Current and valid LPN license in the state of Washington State.
    • - Minimum of 1year of nursing experience preferred but not required with a focus on behavioral health, geriatrics, or dementia care.
    • - Strong clinical assessment and critical thinking skills.
    • - Excellent communication, interpersonal, and teamwork abilities.
    • - Compassionate, patient-centered approach to care.
    • - Ability to manage challenging situations with professionalism and empathy.
