What are the responsibilities and job description for the LPN position at Cascade Park Communities?
- Play a vital role in delivering specialized care to patients with behavioral health challenges and dementia.
- Assist in planning, implementing, and evaluating nursing care to uphold the highest standards of resident safety and well-being.
- Manage complex cases, offer emotional support, and work collaboratively with a care team to achieve successful outcomes.
- Work autonomously as well as collaboratively with the DRS, HIM, Staffing Care Coordinator, Staffing Coordinator, and care staff (med techs and resident aides).
- Assess patients' daily physical, emotional, and mental health needs.
- Develop, implement, and evaluate individualized care plans in collaboration with residents' families and the healthcare team.
- Administer medications, manage diabetes, monitor patient responses, and adjust care plans as needed.
- Supervise CNAs, HCAs, and other care staff.
- Provide therapeutic interventions and manage crises for patients with behavioral health challenges.
- Assist patients with activities of daily living, ensuring their dignity and respect.
- Educate patients and families on managing dementia and behavioral health conditions.
- Maintain accurate and timely documentation of patient care in compliance with facility policies and regulatory standards.
- Collaborate with physicians, therapists, social workers, and other healthcare professionals for a holistic approach to patient care.
- Participate in community meetings, continuing education, and quality improvement initiatives.
- Current and valid LPN license in the state of Washington.
- Minimum of one year of nursing experience preferred, with a focus on behavioral health, geriatrics, or dementia care.
- Strong clinical assessment and critical thinking skills.
- Excellent communication, interpersonal, and teamwork abilities.
- Compassionate, patient-centered approach to care.
- Ability to manage challenging situations with professionalism and empathy.
