- Conducting detailed health, medical, and social assessments up to one hour long
- Reviewing medical history, diagnosis, treatment, and medication adherence
- Administering and ordering point-of-care and lab tests
- Collaborating with physicians, case managers, social workers, family members, and caregivers
- Providing patient education and follow-up in coordination with Primary Care Physicians
- Master’s Degree or equivalent with NP licensure and prescriptive authority
- Board certification in Adult, Gerontology, Family, or Acute Care
- Current BLS, ACLS, or CPR certifications
- Valid driver’s license and automobile insurance
- Strong communication, computer skills, and ability to work independently
- Flexible scheduling with assessments available days, evenings, and weekends
