Responsibilities Include
- Conducting psychosocial assessments, providing financial counseling, accessing community resources, guiding legal processes, and supporting healthcare professionals.
- Managing caseloads, developing care plans, and monitoring progress to optimize member functioning.
- Facilitating resource access, ensuring proper documentation, analyzing outcomes, and advocating for members.
- Assisting with applications for public programs, providing education and support to members and families, and maintaining compliance with standards.
- Scheduling meetings, completing guardianship paperwork, and performing assessments for long-term care as required.
- Minimum one year of clinical social work experience in healthcare.
- Master's degree in social work (MSW).
- Licensing credentials from multiple states, including required licenses within specified timeframes.
- Experience with Windows-based software.
- Knowledge of community resources and regulatory issues in the service area.
