Position Overview

To advance the Health Partnership’s Community Impact Program, we are seeking an organized, self-starter, with excellent communication skills to serve as our Regional Health Connector and SHIE Outreach Coordinator. 

 

The Regional Health Connector/Social Health Information Exchange (SHIE) Outreach Coordinator plays an integral role in building and strengthening relationships with community organizations and medical and behavioral health providers across the Yampa Valley region. In this role, you will work to improve and coordinate the systems that keep the Yampa Valley healthy and thriving by building and strengthening networks. These networks include supporting primary care, behavioral health, public health, human services and community organizations as well as outreach, education, and engagement to support adoption, effective use, and continuous collaboration within the Social Health Information Exchange. The Regional Health Connector/SHIE Coordinator serves as a bridge between technical systems and community needs, ensuring partners understand the value, functionality, and workflows of the SHIE program.

Supervision Received: The Regional Health Connector and SHIE Outreach Coordinator is based out of the Moffat or Routt County office and supervised by the Community Impact Manager.

 

Supervision Exercised: None.

 

Key Accountabilities:

Develop and strengthen relationships with community partners

  • Strengthen relationships with community-based organizations (CBOs), including public health, SHIE Host Organizations, referral partners, and community stakeholders.
  • Represent THP and share about RHC and SHIE work at meetings, coalitions, community events, and regional or statewide convenings.
  • Plan and support organizational initiatives and priority projects to advance health equity in Northwest Colorado, including marketing and social media.
  • Gather partner feedback and community insights to inform program improvements, workflow refinements, and technical enhancements to the SHIE network.
  • Perform and track partner organization outreach efforts to increase adoption of the SHIE and educate on progress and ongoing enhancements to the SHIE network.

 

Develop and strengthen relationships with clinical and behavioral health providers

  • Increase awareness about The Health Partnership’s programs, including Care Coordination, Recovery Support and Health Insurance Outreach and Education with medical and behavioral health providers.
  • Build and maintain successful relationships with prospective and current medical and behavioral health providers and community partners, providing them with training and coaching as needed, working closely with them to identify needs and gaps, conducting outreach to facilitate their participation in the SHIE.
  • Host behavioral health provider gatherings.
  • Provide education to providers about expanding access to Medicaid covered clients.

 

Increase health equity understanding across the Yampa Valley

  • Maintain county specific health and wellbeing resource lists and disseminate to medical and behavioral health providers and community partners; keep the Yampa Valley Resources website

         up to date. 

  • Send monthly newsletter.
  • Provide and facilitate community trainings and gatherings on THP programs and new resources.

 

Collaborate and coordinate with the Community Impact and SHIE team managers to ensure all program deliverables are being met and program evaluations are conducted

  • Complete monthly reporting and training requirements for Regional Health Connector activities.
  • Collaborate closely with program managers, technical team, and governance group to align outreach efforts with program objectives.
  • Track outreach activities, partner engagement, and participation metrics using designated tools or reporting systems.
  • Support language access and culturally appropriate communication strategies when working with partners and communities.

 

Team Player:

  • Engage in cross-organization efforts, connecting project work to the broader Partnership.
  • Share unique skills and expertise with NCCHP team.


General Qualifications & Requirements:

The Health Partnership is seeking candidates who meet or exceed the following qualifications:

  • Bachelor’s degree in public health, social work/human services, communication preferred, or relevant experience.  
  • Ability to communicate effectively with diverse audiences including community members, clinical partners, health and human service agencies.
  • Experience developing and providing strategic communications including the creation of written and oral content.
  • Experience in community outreach and/or recruitment, desired.
  • High levels of organizational skills with a focus on detail and follow through.
  • Passion for social justice and treating all with equal respect and dignity.
  • Competent with Microsoft Office and associated applications and Google.
  • Familiarity with social media.
  • Possess a Colorado driver’s license and have access to a reliable vehicle – this role requires regular travel within the region.
  • Must be able to lift office equipment up to 20-pounds and climb stairs at site/client meetings.
  • Bilingual/Bicultural applicants are strongly encouraged to apply.
  • Must provide proof of a valid driver’s license and adequate insurance coverage totaling at least $300,000 per occurrence.


Compensation:

This is a salaried, exempt position.

 

$60,000-$67,000 per year.

Salary : $60,000 - $67,000
