- Ensuring compliance with discharge planning regulations and identifying barriers to patient flow
- Assessing high-risk patients for readmission, analyzing root causes, and planning appropriate care transitions
- Participating in quality improvement initiatives and managing utilization, case management, and post-acute care coordination
- Applying evidence-based practices and adhering to professional nursing standards and ethical guidelines
- Graduate of an accredited nursing program
- Minimum of three years' acute care nursing experience
- Valid RN license for the applicable state or a compact license
- Physical ability to perform frequent lifting, standing, walking, and data entry tasks
- Excellent communication skills and attention to safety
- Day shift role with a 32-hour workweek
