Medicaid Homecare Services

Role: Certified Nurse Assistant in the home care setting, responsible for providing direct patient care in clients' residences.

Key Responsibilities

• Assist with personal hygiene, including bathing, shaving, hair, and mouth care.
• Aid in transfers, repositioning, and ambulation.
• Provide basic nursing care and prescribed exercises under supervision.
• Prepare meals considering client preferences and dietary needs.
• Offer companionship and engage clients in activities tailored to their interests.
• Perform light housekeeping duties and maintain accurate records.
• Report changes in clients' physical or psychosocial conditions.
• Complete tasks via EVV app and adhere to the care plan.
• Ensure patient confidentiality, privacy, and safety, reporting any abuse or neglect immediately.
  
  

Qualifications & Skills

• Maintain certification with DFS.
• High school diploma or GED preferred.
• Effective communication skills and ability to follow directions.
• Physical ability to lift approximately 50 lbs, bend, and stand.
• Sympathetic attitude toward patient care.
  
  

Additional Notes: The role involves travel within the designated area, with no guaranteed hours or assignment length. The CNA spends most time providing direct care within clients' homes, following the care plan and using equipment such as blood pressure cuffs and stethoscopes.
