As a full-time Case Manager, you’ll help coordinate care and services for adolescents receiving residential treatment for depression, anxiety, trauma, behavioral challenges, and other emotional and behavioral needs.

You’ll work alongside a passionate clinical team, youth and families, schools, community providers, and other support systems to ensure each young person has the resources and continuity of care they need to succeed.

Summary of Responsibilities
As a Case Manager at Headwaters Behavioral Health, you’ll have the opportunity to make a meaningful difference in the lives of youth and families during an important time in their journey. You’ll help coordinate care, connect youth and families with valuable resources, and ensure services and supports are thoughtfully organized throughout treatment and into the transition home. Working closely with our clinical team, families, schools, community providers, and other partners, you’ll help identify needs, remove barriers, coordinate appointments and referrals, and create smooth, individualized transitions of care. Your compassion, organization, communication, and follow-through will help ensure every youth feels supported, heard, and connected to the resources they need to move forward.

Qualifications

• Bachelor’s degree in social work, psychology, behavioral health, human services, healthcare administration, or a related field, required.
• At least one (1) year of full-time experience working with children in a social service setting, required.
• Experience in behavioral health, case management, care coordination, residential treatment, healthcare, social services, or working with youth and families.
• Strong communication, organization, critical thinking, and follow-through.
• Ability to collaborate effectively with families and multiple professional and community systems.
• Commitment to maintaining confidentiality and professional boundaries.
• Licensed driver with an automobile that is in good working order to get to and from work in various weather conditions to ensure reliability.
