Key Responsibilities
- Lead the patient referral process from review through to acceptance, utilizing CRM for documentation and smooth clinical handoffs.
- Establish strong relationships with hospital referral sources and key decision-makers, aiming for 5-10 new contacts per month.
- Effectively communicate the clinical strengths and competencies of assigned nursing centers to position them as a solution for hospital referral needs.
- Achieve a conversion ratio of 60% or higher for clinically and financially appropriate referrals, continuously driving quality referrals.
- Develop and sustain a market strategy plan for assigned nursing centers, using data analytics to drive key strategic initiatives and meet admission goals.
- RN or LPN licensure preferred, except where mandated by state law.
- Valid driver’s license.
- Five to seven years of recent clinical experience.
