Job Summary
Responsible for assisting with social service functions in the mental health and addictions healthcare setting. Interfaces with community resources in accordance with HIPPA requirements.
Qualifications
Bachelor’s Degree in Social Work or other Social Science Degree preferred.
Two (2) or more years of previous experience in social services, with an emphasis in mental health and addictions work preferred.
Responsibiites
Displays basic knowledge of mental health diagnoses and appropriate treatment techniques.
Attends treatment team meetings to provide and obtain information for case management.
Plans and arranges for patient and family aftercare therapy and psychiatry appointments.
Maintains current information of social and psychiatric services available.
Communicates with families and service providers about patients’ current status and anticipated plans.
Tracks and documents of therapeutic services activities required prior to patient’s discharge.
Works with other members of the therapeutic services department to educate other staff members and community members about mental health issues and therapeutic interventions.
Educates public and other services providers about services available at the Harsha Recovery Center.
Treats patients and families with respect and dignity. Identifies and addresses psychosocial needs of patients and families.
Performs all aspects of patient care in environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
Interacts professionally with patient, families, co-workers, and service providers.
Maintains and respects confidentiality of patient/physician/personnel information.
Demonstrates an ability to be flexible, organized, and function under stress.
Maintains a good working relationship both within the department and with other departments.
Consults other departments as appropriate to collaborate on patient care.
Documentation meets current standards and policies.
Prepares reports and maintains case records.
Promotes personal professional growth through continuing education, seminars/conferences, classes, journal articles and societies.
Must document all communication with families into the electronic medical record.
Arrange for therapy and psychiatric appointments within 7 days of discharge.
Phone calls must be returned within the next working business day.
Other duties as assigned.
