Job Description
Provide clinical social work services to support patients’ emotional, psychological, and social well-being through individual and group interventions.
Essential Duties and Responsibilities:
-Complete 24-hour psychosocial assessments for newly admitted patients.
-Conduct comprehensive psychosocial assessments and develop individualized treatment plans.
-Provide individual counseling and therapeutic interventions.
-Facilitate daily therapeutic and psychoeducational group sessions.
-Monitor patient progress and participate in interdisciplinary treatment team meetings.
-Complete accurate and timely documentation, including 24-hour assessments, treatment plans, progress notes, group notes, individual notes, monthly updates, discharge planning, and all other required clinical documentation.
-Coordinate referrals and community resources as needed.
-Maintain compliance with HIPAA, Medicare, DHCS, and facility policies and procedures.
-Perform other related duties as assigned.
Hours: 9am-3pm, Monday-Friday
Pay: $32.00 - $35.00 per hour
