Job Summary
Crown CMHC is seeking a qualified Master’s Level Social Worker (MSW) to provide social work services in a Community Mental Health Center / Partial Hospitalization Program setting.
The MSW will work with adults experiencing depression, anxiety, psychosis, bipolar disorder, social isolation, functional limitations, family stressors, and difficulty managing daily life needs.
The MSW will assist with psychosocial assessment, treatment planning support, therapeutic group services, case coordination, community resource support, discharge planning support, and documentation as part of the multidisciplinary treatment team.
Responsibilities
- Provide social work services to adult CMHC/PHP clients.
- Assist with psychosocial assessments and identification of client needs.
- Facilitate social work groups focused on coping skills, communication skills, socialization, emotional support, community resources, problem-solving, relapse prevention, and independent functioning.
- Provide supportive counseling and client education within the scope of MSW practice.
- Support development and updating of treatment plans in collaboration with the multidisciplinary team.
- Document client participation, symptoms, progress, barriers, and response to interventions.
- Collaborate with psychiatrist/PMHNP, RN, LCSW, occupational therapist, activity staff, and administration.
- Participate in treatment team meetings as needed.
Requirements
- Master’s Degree in Social Work required.
- Experience in mental health, behavioral health, PHP, outpatient care, community mental health, adult day program, or social services preferred.
- Ability to work with adults experiencing serious mental health symptoms.
- Ability to facilitate therapeutic and psychoeducational groups.
- Strong documentation and communication skills.
- Ability to work as part of a multidisciplinary treatment team.
Preferred Qualifications
- Experience working with adults or older adults in a structured mental health program.
- Knowledge of psychosocial rehabilitation, community resources, coping skills training, discharge planning, and care coordination.
Work Environment
This position is located in a structured outpatient mental health program. Services are provided during daytime program hours.
Pay: $30.00 - $35.00 per hour
