Harmonia Collaborative Care is looking for a compassionate, organized Health Home Case Manager to join our team. You'll help individuals access healthcare, behavioral health services, and community resources while providing ongoing support and care coordination.
What You'll Do
- Meet with individuals in their homes and community settings.
- Develop person-centered care plans and coordinate services.
- Connect individuals with healthcare, housing, and community resources.
- Work with providers, families, and community agencies.
- Maintain accurate, timely case notes and documentation.
What We're Looking For
- Bachelor's degree in human services or a related field required.
- Experience in case management, behavioral health, social services, or care coordination preferred.
- Strong communication, organizational, and computer skills.
- Ability to work independently and manage multiple responsibilities.
- Valid NYS driver's license and reliable transportation for community visits.
- Availability Monday–Friday, beginning at 7:30 a.m.
