What are the responsibilities and job description for the Social Care Navigator position at Harmonia Collaborative Care?
Job Type: Part-time; Schedule: Approximately 18–22 hours per week
Harmonia Collaborative Care is looking for a compassionate, organized Social Care Navigator to help community members connect with services and resources that support their health and well-being.
What You’ll Do- Complete social care screenings
- Connect individuals with community resources and services
- Make referrals and conduct follow-up
- Verify eligibility and maintain accurate documentation
- Communicate with clients, providers and community partners
- Help people navigate barriers to needed support
What We’re Looking For
- High school diploma or equivalent
- 1–2 years of experience in care coordination, healthcare, community services, public health, nonprofit services or a related field
- Strong communication and organizational skills
- Comfortable working with individuals from a variety of backgrounds
- Community Health Worker certification is welcome but not required if other qualifications are met
