Responsibilities
Lead the Social Services department in providing high-quality care and services to our residents, ensuring that their physical, emotional, and social needs are met
Develop, implement, and evaluate individualized care plans in collaboration with the interdisciplinary team
Coordinate and facilitate social services activities, programs, and events that promote resident engagement, well-being, and quality of life
Build and maintain strong relationships with residents, family members, and healthcare professionals to ensure effective communication and collaboration
Identify and address resident concerns, complaints, and issues in a timely and professional manner
Monitor and report on resident satisfaction, quality of life, and social services outcomes to inform departmental improvements and strategic planning
Collaborate with other departments, including admissions, marketing, and quality improvement, to ensure a seamless and comprehensive care experience for residents
