Role Description
This is a full-time hybrid role for an HCBS Coordinator with Hamaspik, based in Queens, NY, with some work-from-home flexibility. The HCBS Coordinator will manage and oversee the delivery and coordination of Home and Community-Based Services (HCBS) to ensure that client needs are met in accordance with established care plans. Key responsibilities include conducting assessments, coordinating services, maintaining accurate documentation, and liaising with clients, families, and service providers. The coordinator will also ensure compliance with federal, state, and organizational regulations and standards.
Qualifications
- Experience in case management, service coordination, or care planning within healthcare, social services, or human services
- Strong organizational, time management, and administrative skills
- Effective communication and interpersonal skills with the ability to build strong relationships with clients, families, and partners
- Proficient in documentation, data management, and familiarity with compliance standards and guidelines
- Ability to work collaboratively with a multidisciplinary team as well as independently
- Knowledge of HCBS programs and guidelines is preferred
- Proficiency in Microsoft Office Suite and relevant software applications
- Bachelor’s degree in Social Work, Psychology, or a related field is preferred
