Summary

We are seeking a dedicated Community Health Worker to join our Baton Rouge team. In this role, you will play a vital part in supporting individuals with disabilities and promoting public health initiatives within the community, aligning with our mission to enhance the quality of life for those we serve.

Responsibilities

  • Screen for and assess each family's environmental risk factor and experience of social/environmental adversity that may negatively affect the health and wellness of both children and caregivers.
  • Conduct outreach and education on public health topics within the community.
  • Assist clients in navigating healthcare systems and accessing necessary services.
  • Collaborate with healthcare providers and social service organizations to coordinate care.
  • Facilitate workshops and training sessions on health-related topics.
  • Collect and analyze data to assess community health needs.
  • Advocate for clients’ rights and access to services.
  • Manage the distribution of flexible funds.
  • Maintain accurate records of client interactions and progress.
  • Qualifications
  • Bachelor’s degree in social work, public health, or related field or five years' relevant experience in community outreach/resource navigation
  • Experience working with individuals with disabilities or in social services (highly preferred).
  • Strong communication skills, both verbal and written.
  • Knowledge of public health principles and practices.
  • Ability to conduct motivational interviewing techniques effectively.
  • Familiarity with data collection methods and medical terminology (preferred).
  • Proficiency in public speaking and community engagement strategies (preferred).
  • Certification or training related to community health work is a plus.
