Position Summary
The Case Manager serves as a key member of the multidisciplinary treatment team, responsible for coordinating care, supporting discharge and aftercare planning, and advocating for the individualized needs of each resident. This position ensures continuity of care from admission through discharge, while maintaining compliance with all BSAS (105 CMR 164), DPH, and Joint Commission standards.
Primary Responsibilities
Maintain accurate case management documentation in the EMR.
Develop individualized case management plans that address social, legal, medical, and housing needs.
Collaborate closely with medical, nursing, clinical, and peer support teams to coordinate care and ensure wraparound services.
Establish and maintain relationships with outside providers, courts, probation/parole officers, and social service agencies.
Coordinate discharge and aftercare planning, ensuring warm handoffs to outpatient treatment, MAT/MOUD providers, and housing programs.
Participate in case conferences, multidisciplinary rounds, and weekly team meetings.
Maintain compliance with confidentiality laws (HIPAA, 42 CFR Part 2) and all facility policies and procedures.
Support residents in navigating insurance, benefits, and transportation to follow-up services as appropriate.
Qualifications
Experience: Minimum 1–2 years’ experience in substance use treatment, behavioral health, or case management.
Strong organizational and communication skills with an ability to manage multiple priorities.
Knowledge of BSAS 105 CMR 164, trauma-informed care, and recovery-oriented systems of care.
Ability to collaborate effectively within a multidisciplinary team.
