Position Overview
As the MDS Coordinator, you will oversee the full RAI process, ensuring timely and accurate completion of MDS assessments while supporting care planning and reimbursement accuracy.

Key Responsibilities
- Ensure timely completion of all MDS assessments
- Maintain accuracy of documentation and regulatory compliance
- Manage MDS transmissions and validate state submissions
- Coordinate and lead interdisciplinary care plan conferences
- Monitor and improve Quality Measures (QM)
- Collaborate with nursing, therapy, and leadership teams
- Ensure care plans reflect resident needs and regulatory standards
Qualifications
- Active, unencumbered RN or LPN license
- Minimum of 2 years of MDS experience in long-term care
- Strong knowledge of the RAI process and care planning
- RAC-CT certification preferred
