About Us:We are currently seeking a dedicated and experienced Social Services Director to join our team and play a vital role in enhancing the quality of life for our residents.
Position Summary:The Social Services Director is responsible for planning, developing, and directing the overall operation of the Social Services Department within the Skilled Nursing Facility in accordance with current federal, state, and local standards, guidelines, and regulations. This position ensures that the medically-related emotional and social needs of residents are identified, addressed, and maintained on an individual basis.
Key Responsibilities:Develop and implement social service programs to support residents’ psychosocial needs within the Skilled Nursing setting
Conduct comprehensive social service assessments and develop individualized care plans
Advocate for residents’ rights while promoting independence, dignity, and quality of life
Participate in interdisciplinary care plan meetings
Serve as a liaison between residents, families, nursing staff, and administration
Provide counseling and support to residents and families
Assist with admissions, discharge planning, and transitions of care within the Skilled Nursing continuum
Ensure documentation is completed in compliance with Skilled Nursing regulatory requirements
Bachelor’s degree in Social Work or related field (preferred)
Minimum of 2 years’ experience in a Skilled Nursing Facility or healthcare setting
Knowledge of federal and state regulations governing Skilled Nursing Facilities
Strong communication, organizational, and problem-solving skills
Compassionate, resident-centered approach
