What are the responsibilities and job description for the Patient Care Coordinator position at GREATER ROCHESTER ORTHOPAEDICS, P.C.?
Works directly with the physicians and physician assistants. Prepares patients to see the physicians or physician assistants by assessing the reason for the visit and documenting past and present medical history. Facilitates diagnostic tests and paperwork as needed and assists the physician with clinical duties as directed. Assists with transport and maintains consistent workflow. Keeps treatment rooms orderly by stocking supplies and cleaning rooms.
Care Coordinator duties:
- Patient care responsibilities as assigned, which may include scheduling and organizing patient flow,
accompanying patients to exam/procedure room, assist patients with changing to exam gowns, and transfer to
exam table if needed.
- Read notes so as to know what is expected at the visit.
- Keep patients informed of provider status.
- Assess patient’s general condition and collect patient history.
- Document health history, activities, medication dispensed/prescribed, etc., in the medical record
- Provide basic patient care as ordered by physician or physician assistant with various procedures.
- Cleans rooms after each patient
- Provides patient education materials, communicates physician advice/instructions to patient.
- Obtain lab/x-ray reports
- Schedule MRI, CT scans, labs, etc.
- Clarifies with patient at end of visits if all their questions have been answered
- Schedule referrals to other physicians
- Clean and stock exam rooms
- Adheres to infection control/safety guidelines and confidentiality policies
- Compliance with HIPAA, OSHA and Red Flag Rules
- Other duties and responsibilities as assigned
Qualifications:

- Ability to triage patient care questions related to post-op, post-injury, infection, complications, etc.

- Must have a flexible work schedule with the ability to work late some days and work through the lunch break
on busy clinic days
Requirements:
- High school diploma or GED certificate
- Certified Medical Assistant is preferred, but not required
- Good communication skills
- Demonstrate good organizational skills
