What You'll Do
The Patient Referral Coordinator oversees and coordinates patient referrals to ensure timely, accurate, and seamless care and follow-up. This role works collaboratively with providers, patients, and external partners to facilitate the referral process, maintain precise documentation, and promote continuity of care across the patient journey.
- Coordinate and track patient referrals from initiation through completion, ensuring timely and seamless care
- Verify patient demographic, insurance, and clinical information, and obtain prior authorization as needed
- Communicate with patients to explain referral details, next steps, and assist with scheduling appointments and follow ups
- Maintain accurate documentation in the electronic medical record, including referral status and related communications
- Collaborate with providers, clinical teams, and external partners to support effective care coordination and continuity of care
- Performs a variety of tasks and other duties as assigned
Who We're Looking For
We’re seeking a detail-oriented, adaptable professional with strong communication skills who thrives in a fast-paced environment. The ideal candidate is comfortable with EMR systems, works well with others, and is committed to accuracy, confidentiality, and a patient-centered experience.
- High school diploma or equivalent preferred
- 2-3 years of experience in a clinic setting preferred
