Salary: $115,000 to $120,000 annually comprehensive benefits
We are looking for a Licensed Clinical Social Worker (LCSW) to provide remote mental health support to patients in Oregon. The position involves completing psychosocial assessments, identifying patient needs and risk factors, monitoring outcomes, and contributing to individualized care plans. The LCSW will work with patients, families, and members of the care team to support appropriate interventions and continuity of care.
Responsibilities
- Conduct interviews with patients and families and complete psychosocial assessments
- Identify high-risk patients and assist with appropriate intervention planning
- Monitor patient progress and document outcomes in relation to established care goals
- Participate in the development of patient-centered care plans
Requirements
- Master’s degree in Social Work.
- Valid Oregon LCSW license.
- Demonstrated expertise in clinical assessments and care coordination.
- Strong communication and teamwork skills.
