- Build and maintain strong SNF referral relationships
- Drive hospice admissions and census growth
- Educate SNF staff on hospice services and eligibility
- Coordinate referrals with intake and clinical teams
- Track referral activity and admissions performance
- Maintain strict compliance with hospice referral regulations
Required
- Hospice or home health liaison / business development experience
- Existing SNF referral relationships
- Proven admissions production
- Strong understanding of hospice compliance
- Experience in the DFW / Tarrant County market
