Street Outreach Coordinator

The Street Outreach Coordinator, based at Our Place Drop-in Center, is a key member of the Strategic Outreach Peer Specialist (STOPS) Campaign, working to reduce opioid use disorder, stimulant use disorders, and overdose deaths in Lewis and Clark County. This full-time, field-based role delivers peer support, crisis intervention, and warm hand-off services to underserved populations—including unhoused individuals, rural residents, justice-involved persons, veterans, pregnant women, and those with trauma or disabilities—connecting clients to services and ensuring quality follow-up and reporting. Strong communication, empathy, and organizational skills are essential. This is a lead position within this program.

Essential Functions

Professionalism

• Arrives dressed and prepared for professional, field-based work (proper footwear, gloves, weather-appropriate gear).
• Maintains good hygiene, presenting a clean and well-groomed appearance.
• Uses professional, respectful language and tone with clients, partners, and coworkers, even in difficult situations.
• Promptly alerts supervisor to safety concerns, supply needs, or service barriers.

Outreach and Peer Support

• Conducts street outreach in non-traditional field settings (encampments, parks, libraries, bus stations) to engage individuals with OUD or co-occurring conditions.
• Provides peer support, crisis intervention, and harm reduction services (including Naloxone administration) to build trust and connect clients to treatment and recovery services.
• Targets STOPS Campaign priority populations, including unhoused individuals, rural residents, justice-involved persons, veterans, pregnant women, and those with trauma or disabilities.
• Facilitates warm hand-offs to community partners (e.g., Pureview Mental Health, AWARE, Many Rivers Whole Health, United Way of Lewis & Clark County) for mental health, treatment, housing, and social services.
• Conducts jail visits to improve justice-involved individuals’ access to recovery services.

Service Coordination

• Conducts intake evaluations and needs assessments to identify client barriers to recovery, housing, and benefits.
• Creates client service plans linking to treatment referrals, peer support, housing navigation, employment, and education, with ongoing follow-up.
• Coordinates with the Outreach Services Navigator to ensure continuity of care, especially for clients transitioning to recovery or housing.
• Transports clients as needed to medical, treatment, or service appointments.

Data Collection and Reporting

• Tracks outreach encounters, client demographics, services provided, and outcomes for grant reporting.
• Maintains accurate records (contact forms, surveys, sign-in sheets, referral logs) and compiles monthly reports.
• Submits monthly MOAT-funded capacity data to the Lewis & Clark County Metro Region Opioid Governance Committee by the 15th of each month.
• Participates in focus groups and program evaluations to assess client feedback and inform program improvements.

Community Engagement

• Builds relationships with community partners, nonprofits, and local agencies to strengthen service coordination and client advocacy.
• Represents Our Place and the STOPS Campaign in media and grassroots efforts to raise awareness of OUD prevention and recovery services.
• Advocates for policy changes to address stigma and barriers faced by unhoused and addicted individuals.

Maintenance and Safety

• Follows all safety protocols during field outreach, including proper use of harm reduction tools and PPE.
• Reports incidents, accidents, or injuries immediately to the Outreach Program Manager, following program procedures.
• Maintains a clean, safe outreach vehicle and tracks mileage and fuel costs.
• Complies with lifting policies, handling objects weighing up to 50 lbs during outreach activities.

Start of Shift/End of Shift Duties

• Start of Shift: Prepares outreach supplies (Naloxone, program materials), reviews daily locations, and coordinates with the outreach team.
• End of Shift: Documents outreach encounters, organizes supplies, and prepares data for reporting; assists with closing procedures as directed.

Additional Responsibilities

• Attends required trainings, including Naloxone administration and peer support certification, to maintain compliance with grant requirements.
• Participates in monthly meetings, focus groups, and site visits as requested by MOAT or the Outreach Program Manager.
• Covers for team members during breaks or absences, supporting Our Place staff and the outreach team as needed.
• Performs other duties to support the STOPS Campaign and Good Samaritan Ministries’ mission.

Qualifications

• Ability to complete training in Naloxone administration, crisis intervention, and outreach navigation.
• Strong communication and interpersonal skills; able to build trust with diverse, underserved populations.
• Excellent organizational skills and attention to detail for accurate data collection and high-quality reporting.
• Ability to make sound, fast decisions in high-pressure field environments.
• Physical capability to lift objects up to 50 lbs, navigate varied terrains (e.g., encampments, rural areas), and travel within Lewis & Clark County.
• Valid driver’s license and reliable transportation.

Required: High School Diploma or GED.

Preferred: Associate’s Degree in Social Work, Human Services, Psychology, Public Health, or related field; OR Certified Peer Support Specialist or equivalent training in case management/navigation. Lived experience with recovery, homelessness, or OUD is a plus.
