We invite you to join our dynamic team as a Family Support Coordinator. In this vital intake role, you will serve as the welcoming voice and initial point of contact—compassionately guiding families raising children with developmental disabilities or delays, assessing their needs, and connecting them to life-changing resources.

POSITION SUMMARY:The Family Support Coordinator serves as the vital first point of contact and advocate for families raising children with developmental disabilities or delays. In this multifaceted role, you will lead the intake and assessment process, collaborating closely with multidisciplinary teams of professionals to determine eligibility for Early Intervention services. Beyond initial enrollment, you will empower families by coordinating vital resources, delivering tailored support, and managing referrals to essential community programs. Additionally, you will champion the agency's mission in the community by leading Child Find screening events, building professional networks, and driving public awareness initiatives across our service areas.

 

ESSENTIAL RESPONSIBILITIESIntake, Assessment, & Referral Management

  • Overseeing referral processing to ensure all incoming files are complete, accurate, and contain the necessary documentation required for eligibility determinations.
  • Conducting comprehensive intakes for children referred to Family Outreach services, including completing compliance paperwork, educating parents on their procedural rights, and facilitating developmental assessments to help establish eligibility.
  • Managing follow-up communication and support coordination for families whose children are found ineligible, or who choose to opt out of formal services.
  • Maintaining consistent contact with waitlisted families, ensuring their needs are monitored and that internal contact logs are kept meticulously up to date.

Team Collaboration & Data Integrity

  • Partnering with multidisciplinary teams of professionals to gather necessary supporting documentation (such as medical records or evaluations) to streamline the eligibility process.
  • Ensuring exact data entry of all required client documentation into the State Data Management system in compliance with state standards.
  • Maintaining secure, confidential electronic health records for all referrals utilizing Family Outreach’s internal data tracking systems.

Community Outreach, Child Find, & Public Relations

  • Coordinating and conducting developmental screenings at community Child Find events to proactively identify children who may benefit from Early Intervention services.
  • Cultivating strong professional relationships and networking with community providers, medical professionals, and agencies within related service fields.
  • Representing Family Outreach at public meetings and community tables, including actively participating in the Early Childhood Coalitions.
  • Executing targeted public relations initiatives, including delivering informational presentations to outside agencies, distributing annual awareness materials, and coordinating special projects outlined by the PR subcommittee.

Professional Engagement & Agency Support

  • Providing temporary caseload coverage for fellow Family Outreach team members when necessary to ensure continuity of care for families.
  • Participating actively in all required professional development trainings, team meetings, and Early Intervention staff briefings.
  • Performing additional administrative and professional duties as assigned by the supervisor to support the overall mission of the agency.
Qualifications:

KNOWLEDGE, SKILLS, & ABILITIES:

  • Basic knowledge of developmental milestones
  • Ability to write objective case notes
  • Professional and effective communication in all settings
  • Ability to be well versed in Family Outreach operations
  • Skill in the operation of general office equipment
  • Knowledge of general community resources
  • Skills in building public relations
  • Accurately processing forms
  • Organizing and prioritizing work
  • Follow verbal and written instructions and procedures
  • Research and assess family needs
  • Design informational and educational materials, methods and learning activities

RECOMMENDED QUALIFICATIONS:

  • A minimum of a bachelor’s degree preferably in a related field such as Early Childhood Education, Sociology, or Psychology.
  • Recommended two years of related experience including service coordination, child education, resource allocation.
  • A valid MT driver’s license or the ability to obtain a license within one month of hire, have access to a reliable vehicle, an acceptable driving record.
  • Candidates must also successfully complete a background check and criminal history review.
