Goldie Floberg QIDP's do not work as DSPs or Supervise DSPs. On-call hours are not covering DSP hours, only medical emergencies that can't be covered with DSPs.
Position Summary: This position works with the people served to help them create a life of their choosing and to help them advance toward achievement of their personal goals and desired outcomes. Under the direction of the assigned QIDP, this position will assist with facilitating the coordination of all program services that affect the life of each individual while ensuring case regulatory compliance.
Schedule: This is a full-time position that requires a staff member to work a minimum of 80 hours per two-week pay period. The Case Manager works primarily during office hours (8:30 a.m. – 5:00 p.m.) with flex scheduling, refer to policy 502 Work Schedules for details. Operational needs will require working more than 80 hours per pay period at times and working outside of office hours at times.
The Case Manager spends a significant component of their time working hours in the field directly with individuals and Direct Support Professionals. This time would accumulate to average a minimum of 25% of working hours. Achieving this may involve working weekly evening hours and monthly weekend hours.
The Case Manager shares a rotating program on-call that will require assisting people served and DSPs, during and after business hours and on weekends in accordance with policy 109 Program Team on-Call Procedures.
Remote Work/Telecommuting: Remote Work is approved for this position in accordance with policy 502 Work Schedules.

Essential Requirements:
- Must possess a bachelors degree in specific field.
- Must be at least 21 years old.
- Must possess a valid driver’s license and have reliable transportation. Must maintain eligibility to be a Goldie Floberg (GF) approved driver (DCFS, DHS, and Insurance approval clearance).
- Must meet all of the requirements and be able to perform all of the essential job functions of the DSP position.
- Must complete and maintain all DSP training requirements including annual re-training and medication administration privileges.
- Must take and successfully pass the S-TOFHLA reading comprehension test.
- Demonstrate sufficient communication and writing skills to complete required documentation.
- Must successfully pass all required background checks and not be convicted of any crime that violates DHS or DCFS requirements for employment with Goldie Floberg.
- Must be able and willing to work with individuals who exhibit contextually inappropriate behavior, including, but not limited to, physical aggression towards staff and/or people served, self-injurious behavior, and verbal aggression across a variety of environments including home and community settings.
- Must be able and willing to work with individuals who need significant or total physical care for activities of daily living including, but not limited to, bathing/showering, toileting (including wiping), eating, and dressing across a variety of environments including home and community settings.
- Must be able to complete data entry using a laptop, computer or tablet.
